某天深夜，不良博爱医院的内科医生辛玉廷（马崇乐）和画匠老方救下一名被日本宪兵队追捕的伤员后，不良另一名医生周益（雷明）向外挂了一通神秘电话。不久，日本宪兵队特高课青木（袁春海）收到号称“帝国军人眼睛”的“四十五号”提供的情报，下令搜查博爱医院，翻译官丁彦（郭法曾）首当其冲搜查时，扑了空。“四十五号”现身日本宪兵队，称周益就是要找的新四军的买药人，原来他是……知晓药品交易的时间和地点后，青木再派丁彦出发，殊不知丁彦同“四十五号”的身份并无差别。
